PLUS Annual Base Fee	$750.00 to open a Subaccount
Annual Asset Value Fee	1% of the market value of the account
Annual Tax Prep Fee	$39.00
Final Tax Prep	$75.00 based on closing acct

Annual Bank Management Fee
Pooled Trust	0.45% of the market value of the account

Fees are for usual and customary services including administration of the account, investment management, securities processing and custody, record keeping and other fiduciary services.

The Annual Fee is charged annually at the anniversary of the opening of the account. The Annual Asset Value Fees are charged quarterly. Additional charges will be made for extraordinary services (including tax preparation, attorneys’, accountants’ and other advisors’ fees) provided. Expenses are charged as incurred.

Services are terminated by insufficient funds, death of a beneficiary or by court order.

When more than one trust is created under a single document, fees are applicable to each account separately.

All fees and charges are subject to change upon written 90 days notice.

NOTE: All fees are paid out of the Trust Subaccount.
